The payer could not verify a required order, its elements, its timing, or the ordering provider's eligibility for the billed item or service.
First checks
- 1Identify the policy requiring the order.
- 2Check date, signature, item or service, quantity, and ordering provider.
- 3Compare claim ordering data with the record and enrollment source.
Resolution path
- 01Correct claim ordering data when the valid order was present.
- 02Submit the existing timely order through reopening or appeal when allowed.
- 03Do not create or backdate an order after the fact.
- 04Fix intake controls for future services.
